By booking your appointment you acknowledge you agree to the general terms and conditions of West Wales Ultrasound and Chiropractic Clinic (which also pertain to our sister clinic, Tenby Chiropractic) and you also agree to our booking and cancellation terms and conditions detailed below (also available on our website):
Booking Policy:
All appointments should be made through our online platform.
For ultrasound scanning appointments full payment is required at the time of booking.
Initial consultation chiropractic appointments whether in Carmarthen or Tenby require a non-refundable deposit at the time of booking, with remaining monies to be paid at the time of the appointment. All prepayment transactions will be conducted exclusively online through our secure payment gateway.
For ultrasound appointments: If you are attending West Wales Ultrasound and Chiropractic Clinic in Carmarthen and you are not being referred by a healthcare professional (i.e. you are self-referring), you must first pay for and attend an initial consultation, via our online booking portal. Any subsequent scanning then performed during the appointment must be paid immediately via cash or contactless card transaction. If scanning is not undertaken following our clinical assessment you will not be required to pay any further charges. All scanning requires written patient consent and this will be taken prior to scanning.
If you are being referred to us for ultrasound scanning from an external source, the referring person must be both currently licensed and from a regulated healthcare profession. This includes, but is not limited to, physiotherapists, podiatrists, chiropractors, osteopaths and medical doctors. A referral form from West Wales Ultrasound and Chiropractic Clinic's website must be completed and submitted to us by the healthcare professional prior to the time of your scan. We strongly advise you to obtain confirmation from your referring healthcare professional that a referral form has been sent to West Wales Ultrasound and Chiropractic Clinic before placing your booking. West Wales Ultrasound and Chiropractic Clinic (including under the trading name Tenby Chiropractic) reserves the right to cancel bookings, with forfeiture of the full booking fee, if the appropriate referral forms have not been received prior to commencement of the appointment.
West Wales Ultrasound and Chiropractic Clinic (and it's sister clinic Tenby Chiropractic) will always strive to facilitate a friendly and effective service. Forfeiture of the booking fee will usually only be implemented after efforts have been made by us to rectify any discrepancies.
In cases of referral to us for ultrasound services, however, any bookings without the corresponding referral form will be subject to cancellation and penalty of the full booking fee at West Wales Ultrasound and Chiropractic Clinic's discretion.
Patients to all of our clinics are required to arrive to their appointment on time. If a patient arrives late, we reserve the right to cancel the appointment with forfeiture of the full booking fee, and no refund will be given.
However, we do understand that sometimes unforeseen circumstances may delay a patient's arrival and cause them to arrive late; whenever possible, we will conduct the appointment as planned, or reschedule without charge. West Wales Ultrasound and Chiropractic Clinic (and Tenby Chiropractic) reserves the right to determine if a late start will be conducted as planned without penalty or if the appointment must be cancelled with forfeiture of the full booking fee.
Cancellation Policy:
We understand that unforeseen circumstances can arise, and we strive to provide flexibility and fairness for both our clients and our clinic. Please review our cancellation policy below:
1. Patient Cancellations: To receive a full refund for your appointment, we kindly request a minimum of 48 hours' notice of cancellation. This allows us to accommodate other patients who may be in need of our services. If you cancel with at least 48 hours' notice, your payment will be refunded in-full.
2. Patient Absence or Late Cancellations: If you fail to cancel your appointment with less than 48 hours' notice or do not show up for your scheduled appointment, you will forfeit the appointment fee, and no refund will be issued.
3. Clinic Cancellations: In the rare event that West Wales Ultrasound and Chiropractic Clinic, or Tenby Chiropractic, must cancel your appointment for any reason, we will issue a full refund without any deduction. We understand that your time is valuable, and we respect your commitment to your health.
